The name “Continuity” came from the idea that people who are experiencing health care changes should be able to continue living independently with as much freedom, comfort, and dignity as before any health problem began.
Continuity Home Health Care was established in 1996 as an affiliate of T.P.F. Nursing Registry, Inc., an agency staffing New York hospitals with nursing personnel. Timothy P. Ferguson founded TPF Nursing Registry, Inc. (TPF) in 1989 and has been involved in health care staffing ever since. In 1996, T.P.F. Nursing Registry, Inc. opened Continuity Home Health Care, Inc. (CHHC) a licensed home care service agency (LHCSA), in effort to meet the needs of individuals who needed care post surgical care or supportive care following an illness. Coming from a family of health care professionals, I see the need for quality and oversight. Having a loved one ill and trying to understand the myriad of medical probing, individuals are overwhelmed. Having a health professional at your side advocating like my parents did for their clients is inherently important.
To provide New Yorkers an alternative to a nursing home or institutional living. Delaying long term care and remaining in home is critical when trying to regain physical and mental health. Through the intervention of skilled nursing, certified Home Health/Personal care aides, or companion care. To preserve one’s independence during healing or end of life is Continuity’s mission.
Because Continuity Home Health Care, Inc. is a Licensed Home Care Service Agency (LHCSA) most private insurance companies will reimburse costs of care directly to you the insured. We do not accept Medicare or Medicaid. A Consent for Care form will document type of caregiver, the cost, and the estimated duration of care. Continuity Home Health Care, Inc. provides staffing and home care services 24 hours a day, seven days a week. Regular office hours are Monday- Friday 8:00 a.m. – 6:00 p.m.
